Background:Finasteride is an FDA-approved prescription medication for the treatment of benign prostatic hyperplasia (BPH) and male pattern baldness. The drug has been associated with significant risks, including a 1.6% (6 million prescriptions) increase in the use of finasteride over the 5-year period of the previous year. This study evaluated whether finasteride use is associated with a higher incidence of adverse events and mortality in men with BPH, a potential risk of.
Methods:A retrospective cohort study was conducted at the Department of Urology of the First Affiliated Hospital of Sun Yat-Sen University (Si Y. S., Shandong, China) between February 2009 and July 2012. Patients with BPH, a primary diagnosis of benign prostatic hyperplasia (BPH) and male pattern baldness who were newly diagnosed with BPH or male pattern baldness were identified via the medical records of the department, and the patient's medical records were reviewed for the presence of prostate cancer, prostate biopsies, and biochemical parameters. The demographic and clinical characteristics of the patients included the patients' age, gender, the duration of BPH and the number of men with BPH. The medical records of the patients were then reviewed for the presence of prostate cancer, prostate biopsies, prostate cancer, and serum prostate-specific antigen (PSA) level. The prostate cancer, prostate biopsies, serum PSA level, and the clinical parameters of the patients were also obtained from the patients.
Results:A total of 12,061 patients were identified in the study period. The median age of the study population was 55 years (range: 17 to 77 years). The median number of men with BPH and the duration of BPH varied from 3 to 19. The mean prostate cancer incidence (95% confidence interval) was 3.3 (0.6) cases per 100,000 men, and the mean number of men with BPH and prostate cancer was 2.7 (1.0) and 1.3 (1.0), respectively. The mean number of men with BPH with serum PSA level was 1.6 (1.0) (95% confidence interval: 0.9-2.3) and the mean number of men with serum PSA level was 2.1 (1.1) (95% confidence interval: 1.1-3.1). The mean number of men with serum PSA level less than or equal to 2.5 was 0.5 (0.5-1.0). There were 7 cases of prostate cancer and 1 prostate biopsy in the patients with BPH and prostate cancer. The most common treatment in the patients with BPH was finasteride (90.3%), followed by minoxidil (90.3%), and the most common treatment in the patients with BPH was finasteride (80.0%), followed by minoxidil (72.2%) and finasteride (66.8%). In the patients with prostate cancer, there were 6 cases of men with BPH with serum PSA level less than or equal to 1.0 (95% confidence interval: 0.9-1.3).
Conclusion:Finasteride use is associated with a higher incidence of prostate cancer, prostate biopsies, and serum prostate-specific antigen (PSA) level, and a lower mortality than other treatments. The results of this study demonstrate that the use of finasteride is associated with a higher incidence of prostate cancer, prostate biopsies, and serum PSA level.
